Request for Permission to be Absent from Regular Duty
(to be used by unclassified faculty and administrators)

This form shall be filled out and presented for the consideration of the college president or his designee prior to absence from campus which is not regular duty, except that in cases of illness or other unforeseen emergencies the employee shall call in as soon as possible, and shall complete this form on the date of return to duties.
